Preseason Athlete Forms

What paperwork and forms need to be completed in order to practice and compete with the team?
In order to participate in any sport at Lincoln High School you will need to complete the forms in the Athletic Participation Packet. Forms Can Be Found Here.
A physical examination will need to occur before your child may participate in practices.  If you have not done so, please schedule these with your doctor as soon as possible. July and August time slots fill up fast.
