COVID-19 Related Request for Adjustment to Working Conditions

Greetings LPS Staff,

Thank you to everyone who participated in the anonymous survey sent June 19 to all LPS employees about reopening school and the return to work.

The CDC has recently updated the list of conditions that may place individuals at an increased risk of severe illness from COVID-19 (updated June 25th). New conditions that may place individuals at an increased risk have been added, and some conditions identified before were removed. For the next step in this process, any employee who is requesting accommodations and/or adjustments to working conditions based on the CDC list of conditions released June 25 related to COVID-19 needs to please fill out this confidential form so we can begin processing your request. Completion of this form in a timely manner is very important for planning purposes as we may not be able to address any later requests for adjustments/accommodations in a timely manner. If applicable, we are asking that you complete this form by Friday, July 10, 2020.

Please note:

  • This form is voluntary and only to be utilized by staff who will be requesting employment accommodations and/or adjustments related to working conditions based on the CDC list of conditions released June 25, 2020 related to COVID-19.
  • If you will not be requesting an accommodation/adjustment you will NOT need to complete this form.
  • The vast majority of positions in LPS require regular, dependable in-person attendance. Requests to work remotely may not be able to be granted.
